Home Help

Personal Care
Our dedicated team of support workers offer kind, compassionate and discreet personal care in your home. We can provide assistance with:​

Bathing, showering, dressing and personal grooming

Eating and using utensils

Travelling to and from appointments and shopping or social outings where assistance with mobility is required.

In-Home Respite
If you provide regular care and support for a loved one, it is important that you also have time away to attend to your own appointments, participate in social activities or just have a break.
A RangeCare support worker can come to your loved one’s home to provide essential in-home support.

End of Life Care
End of life care is about more than medical support. It's about ensuring comfort, maintaining dignity, and focusing on what matters most to you.

​​​End of Life Pathway
For eligible clients, the Support at Home End-of-Life Pathway can provide access to additional funding and services when care needs increase due to a life-limiting illness. Designed to support people who wish to remain at home, the pathway enables timely access to enhanced care, equipment, and services. To be eligible, a doctor or nurse practitioner must determine that the person has an estimated life expectancy of three months or less.

The End-of-Life Pathway provides up to $25,000 in funding over a 12-week period, with up to 16 weeks available to utilise the funding where appropriate. This funding can be used for a range of services, equipment, and supports that help people remain safely and comfortably at home while complementing specialist palliative and healthcare services.

Advance Care Planning​
Advance Care Planning helps ensure your values, preferences, and future care wishes are known and respected if you are unable to make decisions yourself. This may include discussions with loved ones, appointing a substitute decision-maker, and documenting your preferences in an Advance Health Directive (AHD).
​
An Advance Health Directive is a legal document that records your healthcare preferences and can identify someone you trust to make decisions on your behalf. You may also choose to complete a Statement of Choices, which outlines what matters most to you, including your care preferences and goals for quality of life.
​
Planning ahead can provide clarity for your family and healthcare team, helping ensure future care decisions reflect your wishes.

Support at Home Packages
If you are over 65 years of age, or 50 if Aboriginal or Torres Strait Islander, you may be eligible to receive government subsidies to help you pay for in-home, respite and social support services.​
​
At RangeCare every Support at Home Package client is assigned their own dedicated Care Partner who works with you to design your very own unique & personalised suite of services.
​
We understand at RangeCare things change from time to time and your Care Partner will fully support you during this journey and assist to facilitate the necessary changes along the way.

Home Modifications
Modifying your living spaces is sometimes necessary to enable you to continue living in your home. Modifications such as wheelchair ramps or grab rails offer elderly people and those with restricted mobility greater independence.
RangeCare can coordinate your home modifications to ensure that any changes are well considered into the future through RangeCare employees and local businesses.

Understanding Home Modifications
Home modifications involve making changes to a home’s layout and fixtures and fittings to enable an older person or someone with disability to safely move in and around their home. Modifications are made to support ongoing independence and to reduce the risk of injury.
Some examples of common home modifications include:

Installing ramps and rails

Widening doorways
-
Altering bathrooms and kitchens (e.g. lowering bench heights)

Our Partners
RangeCare works with a team employees and highly qualified and experienced licenced tradespeople to coordinate home modifications on behalf of its clients. All our employees and partners maintain a National Police Check which means that you can be assured of receiving a high-quality trusted service for you or your loved one.
Home & Garden Maintenance
RangeCare can coordinate household tasks such as lawn mowing, gardening and general home maintenance so you can continue to enjoy living in your home.

RangeCare works with qualified staff and in partnership with a number of local gardening and home maintenance service providers and can coordinate your services as part of a government-subsidised or private-service arrangement.
Trusted Partners
All RangeCare’s gardening and home maintenance staff and partners are highly qualified and experienced and maintain a National Police Check. This means that you can be assured of receiving a high-quality trusted service for you or your loved one.
Garden Maintenance*
RangeCare can coordinate garden maintenance on a one-off basis or as a regular on-going service. The range of services available to clients include:

Edging; and more, for the purposes of health & safety.

Home Maintenance*
RangeCare can coordinate the full range of home maintenance services including:

* Please note: There are government-imposed restrictions on clients receiving garden and home maintenance services with government subsidies; however there are no restrictions for clients who are paying for these service through a private,
